 Please print this form, fill out completely, and mail with your entry fees and a #10 (legal or business sized) Self Addressed, Stamped Envelope (SASE) for return of your model registration numbers. Entrants who do not include SASE will NOT receive registration numbers by mail, but will have to pick them up at the show.  You may also request your numbers by email, or use a region 1 number.

Entry is limited to 100 models per entrant.
